＜抄録＞
ユニバーサル・ヘルス・カバレッジ (UHC) は「すべての人が，生涯を通じて，健康増進から予防，治療，リハビリテーション，緩和ケアに至るまで，必要な医療サービスに，その費用を支払うことによる経済的困難を陥ることなくことなくアクセスできる状態」であり，持続可能な開発目標（SDGs）の中心的な課題である．グローバルな協調の中，近年，必須保健サービスへのアクセスは全体的には改善してきた．しかし，家計の過度な自己負担の増加，地域格差，社会的格差はいまだに残存している．公衆衛生看護（Public Health Nursing）は，住民参加，多職種協働をもとに，基本的な公衆衛生機能として中核的役割を担い，個人・家族・地域に根ざした予防・健康増進・危機対応を通じUHCに大きく貢献している．今年は，国際保健機関（WHO）/ 国際看護師連盟（ICN）が「State of the worldʼsnursing 2025」を公表し，その中でも，UHC実現に向け公衆衛生における看護への期待が示されている．一方で，公衆衛生看護のグローバル・ヘルスへの貢献を促進させ，可視化のための課題も多く残っている．国際的に比較可能なコンピテンシーの枠組みが確立されていないことや，国際的なネットワークが限定的であること，公衆衛生看護の人材データが未整備であること，看護職の政策参画が限定的であること，があげられる．地域で包括的な健康管理や健康危機対応などを科学的根拠に基づき実践する日本の保健師活動は，公衆衛生看護モデルとして国際的示唆を与えることが可能であり，今後は，日本の保健師の実践モデルや人材育成体系を国際的に共有し，世界における公衆衛生看護の UHC 推進への貢献を強化することが求められる．

Abstract
Universal Health Coverage (UHC) is defined as a “state in which all people have access the guality health services they need, from health promotion to prevention, treatment, rehabilitation, and palliative care, throughout their live, without suffering financial hardship due to paying for those services,” and constitutes a central agenda of the Sustainable Development Goals. Globally, in the context of strengthened international cooperation, access to essential health services has generally improved in recent years. However, excessive household out-of-pocket expenditures, as well as regional and social disparities continue to persist. Public Health Nursing (PHN), based on community participation and interprofessional collaboration, plays a core role as an essential public health function and contributes significantly to UHC through preventive care, health promotion, and crisis response rooted in individuals, families, and communities.
“State of the World’s Nursing 2025” was published by World Health Organization/ International Council of Nursing this year, which also highlights the expectations for nursing in public health to achieve UHC. Meanwhile, challenges remain in promoting the global contribution of PHN and making it visible.These include the absence of internationally comparable competency frameworks, limited international networking, insufficiently developed workforce data on public health nurses, and limited participation of nurses in policy-making processes. Japan’s public health nurses (Hokenshi), who implement comprehensive community health management and health crisis management and response, etc. based on scientific evidence, can provide valuable insights as a model of public health nursing practice. It is essential to share Japan’s practice models and human resource development frameworks internationally, thereby strengthening the contribution of public health nursing to the advancement of UHC worldwide.
