地域社会におけるヘルスケアシステムの平常時・発災時・復興期モデルの検討

抄録
平常時・災害時における衛生対策の充実および健康の確保には，地域の災害健康危機管理拠点である保健所を中心とした機能を充実・強化するための人材育成が重要である．著者らは，地域における健康危機管理従事職員等に対する人材育成をテーマとする研究の一環として，すべての公衆衛生行政職員を対象に，職種間の連携充実・強化に基づく人材開発および人員配置のあり方検討を行った．さまざまな職種から構成される公衆衛生行政従事者の災害健康危機管理における役割を明らかにするとともに，異職種間連携強化に必要な要因の分析を加える等によって，地域社会の平常時・発災時・復興期におけるヘルスケアシステムのモデルを示すことができた．地域における災害健康危機管理のいっそうの充実・強化には，総合的防災対策という観点から災害時要援護者の避難支援対策の推進，公衆衛生診断に基づく医療対策をはじめとする組織間調整機能の推進や情報発信の推進が重要である．その実現には，地域の災害健康危機管理拠点における調整・企画・評価機能を組織間連携強化によって充実・強化する必要があり，そのために平常時・発災時・復興期における地域社会のヘルスケアシステムを構築できる人材育成は，今後優先的に取り組むべき重点課題の 1 つと思われた．

Abstract
In recent years, the government has promoted a plan to reduce the number of victims of natural disasters to zero. In addition, consideration has been given to strengthen the plan by conducting disaster and health risk management activities in local areas, mainly through public health centers. We evaluated the process of comprehensive personnel development and distribution of public health administrators, through a series of studies on personnel training for staff engaged in health crisis management in local areas. In an attempt to study health care system models during normal, disaster and reconstruction periods in local communities, we investigated the factors necessary for strengthening cooperation among staff with different types of jobs in disaster and health risk management, and we also clarifi ed the roles of public health administrators comprised of staff with various types of jobs. An important task that should be preferentially performed in the future to further enrich disaster and health risk management in local areas is to promote the construction of the health care system from the viewpoint of comprehensive disaster prevention measures. In particular, measures such as providing support for the evacuation of people who need help in disaster situations, and training personnel, should be promoted.
