＜抄録＞
自治体保健師には，2040 年を見据えて，健康格差をもたらす社会的決定要因を注視し，健康への負の影響を縮小するための地域保健活動の展開が求められている．本稿では，自治体保健師が認識している 2040 年に向けて優先的に取り組むべき（健康）格差の問題に対応する取組について，16 自治体から聴取した結果に基づき，2040年を見据えた地域保健活動のあり方について解説する．

Abstract
In anticipation of the year 2040, public health nurses in local governments are expected to develop community health activities that address the social determinants of health that contribute to health disparities and mitigate their negative impact. This paper outlines the future direction of community health activities by analyzing initiatives aimed at addressing health disparities, as identified by public health nurses, based on interviews conducted with 16 municipalities.
