＜抄録＞
少子高齢・人口減少社会において，住民の生活上の困難や健康課題は複雑化する一方で，フォーマル・インフォーマルな支援の提供者は減少する．そのような中で，多様な主体と連携して誰一人取り残さない地域ケアシステムの構築に取り組むことが公衆衛生看護に期待されている．本稿では，多様な主体と連携しながら誰一人取り残さない地域ケアシステムを構築するための基盤となる公衆衛生看護の視点と方法を整理する．次に，難病保健分野における多様な主体が連携した災害対策の地域ケアシステム構築の事例を紹介する．最後に，人口減少社会で求められる多様な主体の連携による地域ケアシステム構築における公衆衛生看護の課題と今後の展望について整理する．

Abstract
While difficulties regarding life and health issues among community residents have become more complicated in an aging society with a declining birthrate and population, there has been a decrease in both formal and informal support providers. Public health nursing is expected to build a community care system that has diverse actors and which leaves no one behind. In this paper, we describe fundamental perspectives and methods for public health nursing to build such a community care system. Moreover, we introduce the case of building a community care system with diverse actors, for disaster management for patients with intractable diseases. Finally, we discuss the challenges and prospects for public health nursing to build a community care system with diverse actors in a society with a declining population.
