at the National Institute of Public Health during the COVID-19 pandemic

本稿の目的は,国立保健医療科学院で実施する専門課程Ⅲ地域保健福祉専攻科を修了した保健師(以下,専攻科修了者)の,新型コロナウイルス感染症パンデミック下(以下,コロナ下)の地域保健活動報告から,活動方法,学び,提言を整理し,地域保健対策の推進に関する基本的な指針(以下,改正地域保健基本指針),地域における保健師の保健活動に関する指針(以下,活動指針),及び地域保健福祉専攻科養成訓練の内容と照らし合わせて考察し,今後の地域保健福祉専攻科の養成訓練への示唆を得ることである.
方法は,まず令和 4 年 1 月に日本公衆衛生看護学会ワークショップでコロナ下の地域保健活動を報告した 5名の原稿から,「活動方法」「学び」「提言」に関する記述を各々抽出し,記述内容を集約して各々一文で書き表した.次に「活動方法」「学び」「提言」の一文を,各々意味内容の類似するものを分類整理してサブテーマとし,さらに類似するものを整理しテーマとした.
その結果,「活動方法」のテーマは【パンデミックを契機とした地域保健活動の開拓】【平時からの備えの発動】【地域保健活動の原則に則った展開】【本庁機能の継続】が得られた.「学び」のテーマは【公衆衛生看護の原理原則の固守】【国立保健医療科学院での体系的な学びの実装】,「提言」のテーマは【統括保健師の機能発揮に必要な組織体制】【統括保健師育成に必要な条件】が得られた.
専攻科修了者は,活動指針を基に,改正地域保健指針に先んじて,コロナ対応を実施していた.また,活動指針を含む公衆衛生看護の原則を,コロナ対応,及びコロナ下での地域保健活動の継続に応用していた.今後,地域保健福祉専攻科では,種々の指針の先を行く力量,新たな健康課題に対して公衆衛生看護の原則を実践に応用していく力を強化するために,理論と省察を結び付ける方法,及び自らの実践を基に「公衆衛生看護とは何であるか」を熟考し言語化する内容を取り入れることも一考である.
The purpose of this paper is to summarize the activity methods, learning, and proposals from their reports of public health nurses who completed the long-term educational program, Community Health and Welfare Course at the National Institute of Public Health (hereafter, graduates) during the COVID-19 pandemic. Also, this paper examines the basic guidelines for the promotion of public health measures (hereinafter, Revised Basic Guidelines for Public Health), the guidelines for health activities of public health nurses (hereinafter, Activity Guidelines), and the contents of Community Health and Welfare Course, and to obtain suggestions for the future training of the Community Health and Welfare Course.
First, from the manuscripts of five graduates who reported their activities under COVID-19 pandemic at the workshop of Japan Academy of Public Health Nursing in January 2022, we extracted each description regarding “activity methods,” “learning,” and “proposals,” and consolidated the descriptions and wrote them in one sentence each. Next, each sentence of “activity methods,” “learning,” and “proposals” was categorized and organized into sub-themes respectively, each with similar semantic content, and then further categorized and organized into themes with similar content.
As a result, the themes of “activity methods” obtained were [pioneering health activities in the wake of a pandemic], [activating preparedness in normal time], [developing health activities in accordance with the principles], and [continuing the functions of the main office of local government]. The themes of “learning” were [adherence to the principles of public health nursing] and [implementation of systematic learning at the National Institute of Public Health], and the themes of “proposals” were [organizational structure necessary for the functions of the supervisory public health nurse] and [conditions necessary for the training of the public supervisory health nurse].
Graduates responded to the COVID-19 pandemic based on Activity Guidelines and prior to Revised Public Health Guidelines. They also applied public health nursing principles, including Activity Guidelines, to respond to the COVID-19 pandemic and the continuation of public health activities under the COVID-19 pandemic. the Community Health and Welfare Course. requires training that enables the steady acquisition of the ability to stay ahead of various guidelines, and to apply the principles of public health nursing in practice to pioneer new health issues. It is worth incorporating methods that combine theory and reflection, as well as contains for contemplating and articulating “what public health nursing is” based on oneʼs own practice.
