抄録
【目的】香川県における外国出生結核患者の動向を明らかにし,地域保健行政上の課題および今後の対応につき検討した.
【方法】2016-2022 年に結核登録者情報システムに登録された全国・大阪府・香川県の新規結核患者を対象に,外国出生患者と日本出生患者の人口学的特徴,出身国,職業,発見契機,治療成績等を記述疫学的に比較した.
【結果】香川県では外国出生結核患者の割合が年次的に増加し,全国および大阪府より高かった.外国出生患者は20-30歳代の若年層と女性に多く,出身国はベトナム,インドネシア,ミャンマー,フィリピンが中心であった.職業は常用労働者および学生が過半を占め,発見契機は健診によるものが多かった.治療成功率は全国・大阪府より高く,多剤耐性結核およびHIV合併例は少数であった.
【結論】香川県では若年就労者を中心とした外国出生結核患者への対応が重要な課題であり,職域連携や療養と就労の支援体制の強化が求められる.

Abstract
 Objective: The aim of this study was to clarify trends in foreign-born tuberculosis (TB) patients in Kagawa Prefecture and to examine issues and future measures for regional public health administration.
Methods: A descriptive epidemiological study of newly registered TB patients was conducted using the Japan Tuberculosis Surveillance data from 2016 to 2022. The analysis included demographic characteristics, countries of birth, occupations, methods of detection and treatment outcomes of foreign-born and Japan-born patients. We then compared the characteristics of patients nationwide, in Osaka Prefecture, and in Kagawa Prefecture.
Results: In Kagawa Prefecture, the proportion of foreign-born TB patients increased annually, exceeding that in Osaka Prefecture and nationwide. Foreign-born TB patients were predominantly young adults in their 20s and 30s, with a higher proportion of females. Most of them were from Vietnam, Indonesia, Myanmar, and the Philippines. More than half were regular workers or students, and many cases were detected through routine health check-ups. The proportion of successful treatment in Kagawa Prefecture was higher than that in Osaka Prefecture and nationwide. There were few cases of multidrug-resistant TB or TB/HIV
co-infection.
Conclusion: It is important to address the issues of TB among young foreign-born workers in Kagawa Prefecture. Strengthening collaboration with workplaces and enhancing support systems that enable patients’ continuity of treatment and employment are essential.
