Written Pledge for Quarantine
Written Pledge
Those who will be staying at home or accommodations secured by the quarantine station according to the country/region where you stayed before entering Japan, are required to submit a written pledge to the quarantine station. During the period, you will be asked to pledge not to use public transportation and to stay at home.
Please submit the one of the following written pledge to the quarantine station after carefully understanding.

Note
If you cannot submit written pledge, you must be quarantined in an accommodation facility secured by the quarantine station. Violation of the pledge may result in suspension measures based on the Quarantine Law, as well as the following
(1) Japanese nationals may have their names disclosed.
(2) status of residence holder may have their names and nationalities disclosed. In addition, they may be subject to revocation of status of residence and deportation proceedings.<
So far, 145 persons have been publicly notified. (As of January 14, 2022)
