京都府におけるプレコンセプションケアの推進に当たっての教育機関との連携－

＜抄録＞
京都府では，望む妊娠を叶え，予期せぬ妊娠を防ぎ，若年層が自らの望む生き方（ウェルビーイング）を実現できるよう，令和 6 年度以降「きょうとプレコン」としてプレコンセプションケアの取組を集中的に進めている．その背景として，京都府における特定不妊治療費助成の受給者については約7割が35歳以上であり，妊活開始時点で妊娠率が低下している可能性があることや，10 代の人工妊娠中絶が年間約 150 件発生していることなどがあり，妊娠に関する科学的知識を持った上でライフデザインを考える機会を提供することが求められていた．令和6年度からの集中的な取組に先立ち，京都府では平成28年度から，妊娠・出産に関する知識の普及や，若いうちからライフデザインを考える機会を提供するために，助産師会・医師会と連携した学校向け出前講座等の取組を実施してきた．このような継続した取組により，教育現場でのプレコンセプションケア推進の基盤となる教育機関との信頼関係が構築されていた．令和5年12月に改定された「子育て環境日本一推進戦略」において，プレコンセプションケアの推進が重点プロジェクトに位置づけられた．これを受けて，京都府では，医師，助産師，大学教員など多様な有識者で構成されるプレコンセプションケア推進にかかる検討会を設置し，ジェンダー平等，性の多様性，対人関係スキル，健康管理，ライフデザインなど幅広い内容を含む包括的な内容の議論を行った．検討会の議論により，教育委員会と連携の上，体系的な高校生向け教育プログラムを全国で初めて作成した．教育プログラムは，学習指導要領を踏まえつつ，学校現場のニーズに応えるべく，動画やワークを含む実践的な内容としている．教育プログラムの作成に当たっては，モデル授業を府内高校・特別支援学校で実施しており，教員からは「準備の負担が減る」「指導しやすい」と高評価が得られ，生徒からも授業内容の理解度や実生活への活用可能性が高く評価されている．また，性の悩みや予期せぬ妊娠に悩む方への相談支援体制として，既存の相談窓口を統合し，令和7 年 7 月からSNSを活用した総合相談窓口を開設しており，特に若年層から多くの相談が寄せられている．こうした相談支援を実施する上でも，府内全ての高校への相談カード配布など，教育委員会と連携した周知を進めている．こうした取組は，長年の学校との協働実績，関係部局間の共通認識，知事主導の推進戦略改定，有識者を巻き込んだ検討体制により実現したものである．京都府では，今後も，幼児期から社会人まで切れ目のないプレコンセプションケアの推進を図っていく．

Abstract
Kyoto Prefecture is intensively promoting a preconception care initiative called “Kyoto Precon” since FY2024 to support desired pregnancies, prevent unintended pregnancies, and enable young people to achieve their preferred way of life (wellbeing).This initiative is based on the fact that about 70% of recipients of the prefecture’s financial assistance program for advanced infertility treatment are 35 years or older, suggesting that their pregnancy potential may already be declining by the time they start trying to conceive. There are also approximately 150 cases of induced abortions among teenagers each year. These circumstances highlighted the need to provide opportunities for young people to consider their life design based on scientific knowledge about pregnancy.Prior to the full-scale efforts that started in FY2024, Kyoto Prefecture has been implementing outreach lectures at schools since FY2016 in collaboration with the Midwives Association and the Medical Association to disseminate knowledge related to pregnancy and childbirth and provide opportunities for young people to think about their life design at an early stage. These continuous efforts established a foundation of trust with educational institutions, which is essential to promote preconception care in the school setting.The December 2023 revision of the “Strategy to Create Japan’s Best Child-Rearing Environment” sets the promotion of preconception care as a key project. In response, Kyoto Prefecture established a review committee including various experts like physicians, midwives, and university faculty to discuss comprehensive topics such as gender equality, sexual diversity, interpersonal skills,health management, and life design.Based on the committee’s recommendations, the prefecture, in collaboration with the Board of Education, developed the nation’s first systematic educational program for high school students.The program follows the national curriculum guidelines while responding to school needs, offering
practical content such as videos and interactive activities. Model classes were conducted at several high schools and special-needs schools in the prefecture, where teachers evaluated the program highly—citing reduced reparation burden and ease of instruction—and students also gave positive feedback regarding their understanding and the program’s usefulness in daily life.As part of the consultation support system for individuals with concerns about sexual health or unintended pregnancy, Kyoto Prefecture also integrated existing consultation services and launched a comprehensive SNS-based consultation service in July 2025. A significantly high number of inquiries have been received from young people. To support these services, the prefecture is also promoting awareness efforts in collaboration with the Board of Education, such as distributing consultation cards at all high schools in the refecture.These initiatives have been made possible by years of collaborative experience with schools, a shared understanding among the relevant departments, the governor-led strategy revision, and the establishment of a review structure involving experts. Moving forward, Kyoto Prefecture will continue efforts to promote seamless preconception care from early childhood through adulthood.
